Toksoplazmozun diaqnostikası seroloji müayinənin nəticələrinə
əsaslanır.
Hamiləliyin I trimestrində IgM(-), IgG(-) infeksiyasının
olmamasını göstərir. Hamiləliyin 22-24-cü həftəsində və doğuşdan
əvvəl
müayinəni
təkrarlamaq olar.
Təkrar müayinələrdə
serokonversiya qeyd olunduqda, dölün infeksiyaya yoluxmasını
müəyyən etmək məqsədilə amniosentez və PZR müayinəsi aparılır
və bundan sonra müalicə təyin olunur.
IgM(+), IgG(-) olduqda birincili infeksiya diaqnozu qoyulur.
2 həftədən sonra IgG təkrar yoxlanılır. Təkrar JgG müsbət olduğu
təqdirdə müalicə təyin olunur. JgG-nin təkrar müayinəsinin
nəticəsinin (-) olması IgM-nin yanlış-müsbət olduğuna dəlalət edir.

IgM(+), IgG(+) olduqda, IgM-in 3 aydan 2 ilədək müsbət qalması
nəzərə alınaraq, yoluxmanın vaxtı avidlik testi ilə təyin olunur.
IgM(-), IgG(+) olması immunitetin mövcudluğunu göstərir.
Müayinələrin davam edilməsinə və müalicəyə ehtiyac yoxdur (B).
Rutin skrininq – tövsiyə olunmur (B).
Müalicə
Birincili toksoplazmoz diaqnozu təsdiq olunmuş hamilələr və
güman edilən və ya təsdiqlənən anadangəlmə toksoplazmozlu
yenidoğulmuşlar müvafiq mütəxəssislərlə birgə aparılmalıdır (B).
Hamiləlik zamanı baş verən toksoplazmozun müalicəsinin
spiramisin ilə aparılması tövsiyə olunur. Qadın hamiləliyin III
trimestrində yoluxarsa və ya dölün yoluxması hamiləliyin
18 həftəsindən yuxarı müddətə təsadüf edirsə, müalicənin
pirimetamin, sulfadiazin və spiramisin kombinasiyası ilə aparılması
tövsiyə olunur. Hamiləliyin 18 həftəsinə qədər pirimetaminin
istifadəsi tövsiyə edilmir
Hospitalizasiya və izolyasiyaya göstəriş yoxdur.

Antenatal xidmət müəssisəsinin tibb personalı hamiləyə şəxsi
profilaktika barədə məlumat verməlidir:
Qida tam bişirilməlidir
Meyvə-tərəvəz diqqətlə yuyulub təmizlənməlidir
Torpaq və qumla işlədikdə əlcək geyinilməlidir
Torpaqla kontaktdan sonra, pişik ifrazatını təmizlədikdən və
ya hər hansı kontaktdan sonra çirkli əllər ilə ağıza
toxunulmamalıdır
Əllər təmiz yuyulmalıdır
Ç iy və ya tam bişirilməmiş ət məhsulları qidada istifadə
edilməməlidir
Ç iy və ya tam bişirilməmiş (qızardılmamış) ətlə kontaktdan
sonra ağıza toxunulmamalıdır

Hamiləliyin pozulmasına göstəriş
I trimestrdə anadangəlmə toksoplazmozun inkişafı riskinin az
olmasına baxmayaraq, dölün ağır zədələnmələri ehtimalı hamiləliyin
pozulmasına göstərişdir. Anada birincili infeksiya təyin olunduqda
və dölün yoluxması təsdiqləndikdə (amniosentez və USM zamanı
dölün beynində pozğunluqların olması) hamiləliyin pozulması təklif
olunur.
Doğuş – təbii doğuş yolları ilə.
Döşlə əmizdirmə – əks göstəriş yoxdur.

Simptomsuz bakteriuriya – infeksiyanın klinik mənzərəsi qeyd
olunmadan həqiqi bakteriuriyaya müvafiq (ən azı 2 sınaqda
götürülən sidiyin 1 ml-də 100000-dən çox bakteriya) olan
bakteriuriyadır. Simptomsuz bakteriuriya sosial-iqtisadi durumundan
asılı olaraq hamilələrin 6%-də (2-11%) qeyd olunur. Hamiləliyin
erkən müddətində bakteriuriyanın müalicəsi 70-80% hallarda
pielonefritin inkişafının, 5-10% hallarda hamiləliyin vaxtından əvvəl
başa çatmasının qarşısını alır.

80-95% hallarda E.coli aşkar olunur. Az hallarda isə:
Proteus mirabilis – 5%

B group streptococcus – 1-5%
Staphylococcus saprophyticus – 2% aşkarlanır
Hamiləlik zamanı infeksiyanın inkişafına gətirən amillər:
sidik yollarının urodinamikasının pozulması
sidik axarlarının tonusunun və peristaltikasının zəifləməsi
nəticəsində onların şişməsi, genəlməsi
sidik axarlarının sidik kisəsinə çəpinə yox, vertikal açılması
böyümüş və sağa çevrilmiş uşaqlığın və genişlənmiş sağ
yumurtalıq venasının sidik axarlarına, əsasən sağ sidik
axarına təzyiqi

Əhəmiyyət kəsb edən klinik əlamətlər qeyd edilmir. Simptomsuz
bakteriuriyanın sistit və pielonefrit kimi fəsadlaşmaları hamilədə
aşağıdakı hallarla baş verə bilər:

Diaqnostika
Sidiyin ümumi analizi zamanı nitritlər və/və ya bakteriyalar
aşkarlanarsa, sidiyin bakteriolojı müayinəsi tövsiyə olunur.
Diaqnoz – sidik yollarının iltihabi prosesinin klinik əlamətləri qeyd
olunmadığı halda sidiyin 1 ml-də 10

koloniyadan çox miqdarda
bakteriyaların olmasına əsaslanır. Sidik sınaqlarının 30%-ə qədəri
uşaqlıq yolunun mikroflorası ilə çirkləndiyindən sidiyin düzgün
yığılması tövsiyə olunur.
Simptomsuz bakteriuriya zamanı leykosituriya qeyd olunmur.
Rutin skrininq – hamiləliyin 12-15-ci həftəsində aparılması tövsiyə
edilir.
Müalicə
Simptomsuz bakteriuriya qeyd olunduğu halda nefroloqun və ya
terapevtin konsultasiyası məqsədəuyğundur.
Aşağıda göstərilən dərman vasitələrindən biri ilə müalicə tövsiyə
olunur:
Amoksisillin – 500 mq gündə 4 dəfə, 7 gün per os
Nitrofurantoin – 100 mq gündə 3 dəfə, 7 gün per os
Sefaleksin – 250-500 mq gündə 3-4 dəfə, 3-7 gün per os
Amoksisillin/klavulonat turşusu 500 mq/125mq gündə 2 dəfə,
3 gün per os
Alternativ mü alicə – hamiləliyin 37-ci həftəsinə qədər
nitrofurantoin gündə 100 mq per os.
Müalicənin effektivliyinin qiymətləndirilməsi
Müalicədən 1-2 həftə sonra sidiyin bakteriolojı müayinəsi təkrar
aparılır və hamiləliyin sonuna qədər ayda 1 dəfə təkrarlanır.
Persistensiya edən və ya rekurrent bakteriuriyada antibiotikoqrama
əsasən 7-10 günlük müalicə kursu təyin olunur.
Profilaktika
Persistensiya edən simptomsuz bakteriuriya zamanı tövsiyə
edilir:
gigiyena qaydalarına riayət etmək (xarici cinsiyyət üzvlərinin
düzgün istiqamətdə yuyulması, birdəfəlik dəsmalların və
maye sabunun istifadəsi)
