By signing this form, I understand and consent to the collecting, using, and disclosing of my personal information by Global Education institute at Duksung Women's University as part of the Korean/English Language Program application and enrollment process for the following terms and conditions:

A. Mandatory Information
- Name, Birthdate, Nationality, Passport Number, Visa Status, Contact Information (Address, Telephone Number, E-mail Address), Educational Information
B. Optional Information
- Gender, Religion, Marital status, Occupation, Korean language proficiency, Korean sponsor, etc.
2. Purpose and Use of Collected Information
A. Personal Verification, Individual Identification, Prevention of false information, Determination of applicant's intention for the admission, Record Preservation for dispute resolution, Notification of information
B. Release of Data to related institutions for verification purposes
3. Period of Personal Information Release
Once the purpose of release of personal information is fulfilled, all information is to be deleted immediately or encrypted for security intentions
