   You are encouraged to register as soon as possible. Registration is limited to 16 participants. You may fax your completed registration for to Mickie at 517-364-3525.
   

 

3. 
   The registration fee is $ 700.00, and must be received in the Trauma office, to be officially registered. For re-certification the cost is $400.00 and it is a half-day course on the 2^nd day.

   Cancellation received 3 weeks prior to a course will result in a 75% refund.
   
5. 
   Cancellations received less than 3 weeks prior to the course will result in NO REFUND (This includes no shows).
   
6. 
   We reserve the right to cancel any course, with at least one-week notice. Students that are enrolled in the course will be given the option to reschedule or receive a full refund.
